This consent form has been prepared as an annex and an integral part of your current employment contract with you within the scope of processing, storing and sharing the “Personal Data” of real persons regulated by the Protection of Personal Data No. 6698 (Law No. 6698).

“Personal Data” within the scope of this Permission Form and Law No. 6698; shall mean all kinds of up-to-date information, including your information in the job application form and personnel file, as of your employment within the scope of the employment contract. This information can be obtained in written, verbal, audio, video or electronic media.

Our company has the title of “Data Controller” as defined in the Law No. 6698. As the data controller, he/she states that the Personal Data will be processed, recorded, stored, preserved, updated and transferred when necessary. We declare that any kind of transaction will be considered as the processing of personal data.

Our company is based on all relevant national and international legislation, primarily the Labor Law No. 4857, the General Health and Social Insurance Law No. 5510, and the Law No. 6331 on Occupational Health and Safety, and the competent administrations (Social Security Institution, Labor and ÿÿ-KUR Directorate concerned). By applying to our company, in accordance with Article 11 of the Law No. 6698; To learn whether your personal data is processed, if so, the information about it, the purpose of processing and whether it is used in accordance with this purpose and the third parties in the country or abroad to whom the said data is transferred, to correct your personal data if it is incomplete or incorrectly processed, Deletion or destruction of your personal data within

the framework of the conditions stipulated in the article, and notification of the transactions made by our company in this context to third parties to whom the information has been transferred, objecting to a result against you due to the automatic systems and analysis of your personal data exclusively.
